With the Blue KC Care Management app, you have access to programs that can help you reach your personal health goals. That might include managing a health condition, recovering after surgery, having a healthy pregnancy, or taking other steps to feel better. Plus, you can use the app to chat privately with registered nurses, social workers, and community health workers when you need more information or support.†
The Blue KC Care Management app is included in your plan at no extra cost.* And downloading it to your mobile phone or tablet is easy. Here’s a step-by-step guide.

You’ll see a screen that says “Great! We have a match.”
Tap the “Continue” button. You’ll be prompted to choose whose health you’ll be managing. Tap “My own.”

You can find your member ID on the front of your Blue KC insurance card. Enter the last 9 characters of your member ID followed by the 2-digit suffix.
Follow the prompts to finish your registration.
You should hear from your Blue KC Care Management team shortly. The app allows us to deliver a care program that’s tailored to your needs, directly to your mobile device.
It also connects you to your Blue KC Care Management team’s dashboard. That way, your Care Team members can track your progress and provide support.
